This might sound crazy, but the two types have the same functions and and when unhealthy ESFP are identical to an ENTJ. Same thing with unhealthy 7w8 compared to average 8w7, there's little difference! ESFP 7w8 if unhealthy are almost identical to ENTJ 8w7, and I'm trying to figure out what are the tell tale signs and differences!

The problem lies in assumptions. Unhealthy ESFP is not identical to ENTJ.
Inferior functions don't suddenly become dominant, that's not how it works. Unhealthy ESFP Is still ESFP. In fact, the extent to which they overidentify with their Dom function will be even greater than what is expected in a "healthy" state, because healthy state implies a constructive relationship with inferior functions.

Similarly, unhealthy ENTJ will not become ESFP. They will overidentify even more with the Te process neglecting their introverted feeling aspect, which doesn't actually help with being objective/reasonable and turns you into some kind of tyrant that is out of touch with humanity.

Next, 7 and 8 enneagrams also continue to be 7s and 8s, because the reason why they become and stay "unhealthy" is inextricably linked to their respective core motives.
If 7 would've changed into 8 under low health, then that would've also meant change in core motives and the reasons why 7s became unhealthy in the first place, which wouldn't make sense. If 7s became unhealthy due to 7 reasons and stay unhealthy due to them, then 7 motivations continue to influence them. They are still 7s.

Behaviour might change, so, look for motivations behind them and try to understand them instead.

Okay, so I’ve got 2 friends in my life who are ENTJ and ESFP, respectively. Both young women in their 20s who are in my peer group, making them good for comparison.

The best I can describe it is the ENTJ tries to let off pressure at first in an indulgent Se way (a night out, a glass of her favorite wine, a vacation, etc.) Te-Se looping. If that doesn’t relieve the pressure, she then gets a bit “frantic” and uncharacteristically helpless/victimized in explaining herself to others. “Please believe me, this was what I was trying to do, why doesn’t anyone understand me??” Inferior Fi stuff. But the Fi still comes out through her dom Te - so she explains her feelings in a “heady” way. She’s expressing her intense Fi through her intellect, like a lawyer making a passionate defense in a court room.

The ESFP is much more raw in her Fi (sits with her feelings quietly, doesn’t express or intellectualize them) and becomes uncharacteristically removed in her demeanor. She also tends to bury her Fi under Te plans to improve her situation. So she’ll smile and brush off help/attention, say “it’s no big deal” and “it’ll all be okay”, even though I can tell she’s hurting. Then she’ll quietly boss up her life by taking immediate Se action on those Te plans (applying for a new job, joining a gym/club, getting on the dating market again, etc.) Se-Te loop. If those plans cannot be actualized quickly/effectively enough, she goes into a deep, quiet existential crisis thing. Inferior Ni stuff.

So while they’re swapping around the same functions and certainly easy to mix up because of it, the loops are different and the inferior grip stuff is different.

Basically they might look like negative versions of the other. Stressed ENTJ = whiny/moody, indulgent ESFP. Stressed ESFP = emotionally repressed, action-oriented, existential dread ENTJ.

One key element is motivation. ENTJ-- impersonal, success through action. ESFP-- hopelessly personal. Trump is a great example of that "they all love me" etc. ENTJ-- objective; ESFP-- subjective. ENTJ-- long term strategy+end result. ESFP-- perception/reception/impression.
Both appear bold, confident, forceful, but there will be different drives. My 2c.

"Pickles" roll down hill, what basically meant by that was.
If someone has a problem or issue whatever word usage one chooses. The Dom Function primarily role is dealing with usual situations normally for most in such a innate way people don't normally realize they are even using it. When a situation occurs that the Dom Function needs a different angle to look at things for whichever reason. This can cause a backlash to the Dom Function, especially with using the Inferior Function.
With that the individual may push back and blame the inferior function. When the Inferior is causing you to snap out of the Dominant functions grasp on you.
The Inferior is doing what it should be doing but pickles roll down hill. The Dominant Function or the individual is not wanting or unwilling to terms and blame the Inferior.
There's also milder versions such as maturity and ways it's used.
An over indulgence in the Dom can also cause pickles to roll down hill. And cause a commotion with the Dom and Inferior, by the Inferior causing a unbalanced situation, Due to the Dom.
Under normal or typical circumstances balanced or unbalanced the inferior is a good way to help tell the type and to assist someone, yet shouldn't be the only way that is done.
The Dominant can't operate properly or i healthy ways without the Inferior, to long in this situation can cause complications.
This doesn't necessarily mean in a bad way either.
The Inferior Function is in a way like the counter balance weight on the back of a forklift.

I had a few unhealthy ESFP friends and have some new ones, and they were/are all explosive. They will threaten people with all kind of things for little reason and a few minutes later they will forget it and act all playful.
I think unhealthy ESFPs don't care as much to keep their word, they will talk a lot and if left alone, do nothing. They may as well erupt with little to no warning. I am way more careful about what I say because I value what I tell people, whether good or bad.
